How ready is your community to create or improve a Community Prevention System? The two tools below are designed to help answer this question.
One tool is a Readiness Assessment tool, designed to create a description of the community’s demographics and overall contract, as well as a description of the collaborative. The goal of the Readiness Assessment is to provide a complete picture of the community and the community collaboration to inform the development of a broad prevention system.
The other tool is a Collective Impact Self-Assessment, designed to guide to assess the present status of the community collaboration across the five main conditions for Collective Impact. It should inform areas to develop.
Information to complete both tools should be gathered from members of the community collaboration. Working drafts of information from each section as well as the final product should be shared with members of the larger collaboration.
Staff or consultants with Nebraska Children may be available to help facilitate the Readiness Assessment and efforts to increase readiness in specific areas identified by the assessment. The completed Readiness Assessment should also be useful for grant applications and planning future activities.
